 This is just to help, you can obviously have more or less detail depending on what you want:

Form
Please fill out the following form for your requests

Name:(character name)
Quote: if wanted
Style: do you want any transparent parts,
specific shapes, etc
Colorizing: if you would like I can make the entire banner a
specific color
Pictures: whatever images you want me to use
Anything else: anything else you think worth mentioning
like a theme or whatever
